What to include in your request

  • Named insured exactly as it appears on the policy
  • The insured's FEIN, which workers comp carriers use to match accounts
  • Policy number(s) and the policy periods you need
  • Years of history requested (three to five is standard)
  • A signed letter of authorization from the insured if you are not the agent of record
