What to include in your request
- Named insured exactly as it appears on the policy
- The issuing member company shown on the dec page (Utica Mutual, Graphic Arts Mutual, Republic-Franklin, or another group company)
- Policy number(s) and the policy periods you need
- Years of history requested (three to five is standard)
- Your agency name and producer code if you are the agent of record
- A signed letter of authorization from the insured if you are not the agent of record

Common questions
Is Utica National the same as Utica First?
No. Utica First Insurance Company is a separate carrier that is not part of Utica National Insurance Group and is absent from its member company list. If the policy was written by Utica First, request loss runs through Utica First's own portal, UFirst Now, not Utica National's At Your Service.
Can I get Utica National loss runs if I am not the agent of record?
Not directly. Either the insured requests their own claim history, or you submit a signed letter of authorization from the insured along with your request.
